Please fill out the following forms before visiting our clinic. We will also have blank forms at the clinic. Please wear comfortable clothes that can be rolled up to your elbows and knees easily. Each visit typically includes an exam and an assessment of your current condition, and the application of needles. The first visit will typically last up to 90 minutes, and the following visits will generally be 60 minutes each.
